SCF Arizona offers two comprehensive PPO plans and
pays a major portion of the premium cost for medical
coverage for eligible employees and dependents. Both
PPOs use the Blue Cross Blue Shield of Arizona
provider network.

Dental Benefits
SCF Arizona offers two plan options through Delta
Dental of Arizona. Both plans cover preventative,
basic and major dental care. In addition, both plans
cover orthodontia for adults and children.

Vision Benefits
A routine annual eye exam is provided within each
medical plan. A voluntary vision plan is available
to employees with a small premium contribution. The
voluntary vision plan provides for an annual eye
exam and covers a portion of the cost of glasses or
contact lenses.

Life Insurance Benefits
SCF Arizona pays 100 percent of the cost for life
insurance and accidental death and dismemberment
(AD&D) coverage of one year’s annual salary.
Employees have the option to purchase additional
life insurance and AD&D coverage for themselves and
dependents.

Retirement Benefits
SCF Arizona is a member employer in the Arizona
State Retirement System. This is a defined benefit
retirement plan that calculates benefits based on
your average monthly compensation and years of
service in the retirement system. For additional

In addition, SCF Arizona offers a voluntary 457
deferred compensation retirement plan administered
by Nationwide Investment Services. The plan offers several different
investment options ranging from low risk to high
risk.

Time Off Program (TOP)
SCF Arizona understands the need for employees to
have a balance between work and home. TOP allows
employees the flexibility of a paid-time-off bank
that may be used for vacation, sick leave or any
other personal leave. In addition, SCF Arizona
employees also enjoy 10 company-paid holidays per
year.

Team Incentive Program (TIPs)
SCF Arizona employees work in teams. Each team has
quarterly and annual goals it is expected to meet. The purpose of
the SCF Arizona TIPs program is to reward teams and
individuals based on how well goals are met,
resulting in organizational profit and financial
stability.

Continuing Education
SCF Arizona recognizes the value of a highly
trained, professional workforce and encourages its
employees to pursue continuing education and
professional development.
Tuition Reimbursement
Employees are eligible for reimbursement based on
these criteria:
• Undergraduate: Open to all; course of study must
be job or career related.
• Masters: Program of study must be within
current career path.
• MBA: For approved SCF Arizona leaders and
employees with high potential for leadership.
• Executive MBA: Approved for high potential
members of leadership team. Requires approval from SCF
Education Council.
Degree/Certification Bonus Plan
The SCF Arizona Degree/Certification Bonus Plan
provides an added incentive for employees to pursue
their professional development through participation
in certain degree and certification programs.
In-House Training
SCF Arizona also has an extensive in-house training
program that offers career enhancement
opportunities.
Wellness Benefits
Wellness Reimbursement Program
Employees are reimbursed up to $200 each year for their
wellness expenses such as fitness center
memberships, exercise equipment and fitness classes.
Annual Physical Incentive
Employees are eligible to receive a $100 incentive each year for getting an
annual physical examination.
Health Risk Assessment Incentive
Employees who complete an online health
risk assessment receive a one-time $25 incentive.
Tobacco Cessation Program
Employees are eligible to receive up to a maximum of
$200 for reimbursement of the cost of tobacco
cessation products.
Annual Health Screenings
SCF Arizona pays for on-site health screenings for
employees and spouses.
Flu/Pneumonia Immunizations
SCF Arizona pays for on-site flu and pneumonia
immunizations for employees and their family
members.
Wellness Consultations
Each week a representative from the Wellness Council
of Arizona is in our Phoenix and Tucson offices to
meet one-on-one with employees to discuss their
health and wellness issues, take blood pressure and
hold classes of interest.
Fitness Classes
In our Phoenix office, SCF Arizona offers a variety
of fitness classes for employees.
Weight Loss Programs
In our Phoenix office, SCF Arizona offers Weight
Watchers at Work and a Be Lean weight-loss program.
Fitness Center
The Phoenix office has a fitness center with
treadmills, elliptical machines, exercise bikes,
universal weight system and locker rooms with
showers.
Mobile Mammography
Twice a year the Mobile On-site Mammography (MOM)
vehicle is on-site at the Phoenix office. Exams are
free to employees covered by our health plans.
Prostate Exams
Once per year the Prostate On-site Project (POP)
vehicle is at the Phoenix office. Exams are free to
employees covered by our medical plans.

Other Special Benefits
Flexible Spending Accounts
SCF Arizona offers both healthcare and
dependent-care flexible spending accounts.
Short-Term Disability
SCF Arizona's company-paid short-term disability program provides a benefit of
60 percent of base salary.
Long-Term Disability
Through the Arizona State Retirement
System this program provides a benefit of 66 2/3 percent of
salary.
Long-Term Care Insurance
An SCF Arizona-paid benefit that provides $1,000 per month for up to 6 years. Employees have
the option to purchase additional coverage through
group rates.
Pet Insurance
A voluntary pet healthcare program is offered through United Pet Care.
Family Resource Benefits
Child and elder care resource and referral
benefits are available, as is adoption assistance. For the
Phoenix office, this includes back-up childcare,
mildly ill child care and back-up elder care.
Employee Assistance Program (EAP)
SCF Arizona offers paid counseling benefit for employees
and dependents.
Supplemental Health Insurance
Voluntary supplemental benefits are provided through
Colonial Supplemental Insurance.
Savings Bond Program
A payroll deduction program to purchase U.S. savings
bonds is offered.
Automobile/Homeowners Insurance
A payroll deduction program through MetLife
allows for the convenience of payroll deduction for
your automobile and/or homeowners insurance premium.
SCF Employees Association
Provides deals and discounts on tickets, goods and
services.
Alternative Medicine
Discount alternative medicine program providing discounts for a large network of complimentary and alternative medicine providers.
