Q. When will I get my first check?
A. Your check will be issued within 21 days of
reporting if the injury is reported promptly to the
Industrial Commission of Arizona by the physician
and your employer.

A. Temporary Total Indemnity is payable every 14
days.

Q. When I am on light duty, how often do I get my
check?
A. Temporary Partial Indemnity is payable every
30 days upon receipt of the completed Monthly Status
Report sent to you by your claims adjuster.

Q. How much will my check be?
A. Indemnity payments are based on 66 2/3
percent of your Average Monthly Wage. As of Jan. 1, 2008, the statutory maximum is $3,000 per month. On Jan. 1, 2009, the maximum will be $3,600. Thereafter it will be adjusted annually to reflect increases in Arizona’s average annual wage, not to increase more than 5 percent per year.

change of address form to complete, sign and return.
