Q. When are medical bills payable?
A. SCF Arizona makes every effort to process and
pay within 30 days all medical bills with appropriate and necessary information on open
and accepted claims.

Q. How long do I have to submit bills?
A. Dates of service that are greater than three
years old may be denied.

Q. How long do I have to appeal a payment decision?
A. Medical providers have 12 months from receipt
of the Explanation of Benefits to contest and appeal
our decision. You may mail your appeal to:

Q. What billing forms must I use?
A. Outpatient clinics should use the CMS-1500
form and hospitals or surgery centers use the UB-04
form.

Q. Why don’t I get paid the same amount I billed?
A. Reimbursement for services provided by
physicians, physical and occupational therapists as
well as various other providers of healthcare
services are governed by the Industrial Commission
of Arizona’s Physician Fee Schedule.

A. Order the schedule from the Industrial
Commission of Arizona website.

Q. How do I become a provider for Arizona’s
injured workers?
A. To join SCF’s Preferred Connection Network, send by mail or fax a W-9 and letter of interest including your date of birth, specialty and contact person on your letterhead indicating your willingness to participate in treating Arizona’s injured workers and receiving reimbursement. We must have this document before we can process and pay any of your bills within the PCN.

Q. I’m an out-of-state provider and my state’s
laws say you must pay me a certain way.
A. All of SCF Arizona’s claims are administered
under the laws and statutes of the State of Arizona
regardless of where our injured worker lives or
seeks care.
