Reporting an Injury
Please complete the Form 102 with the injured worker.
It should be completed and mailed within eight days of initial treatment. Be sure to fill out the form completely, especially item 14, the first date of treatment and when working disability began; item 18, the ICD-9 code; and item 29, work status. This form, when completed and signed by the injured worker, constitutes a legally filed claim.
Fraud Prevention
As medical care costs skyrocket, fraud gives
unscrupulous providers an unfair advantage over
honest providers. If you suspect a provider of
billing for services that were not rendered,
prescribing unnecessary drugs or treatment, or
falsifying a diagnosis, please do your part to keep
costs in control by reporting it to the SCF Arizona
fraud hotline at 800.526.5226.
Also, if you suspect an injured worker or an
employer is participating in fraud by faking an
injury, its severity, or misrepresenting job
classifications, please contact the hotline at
