Filing a Claim


1. Immediately notify your employer of the accident, injury or industrial illness.

2. Ask your employer for a referral to a physician or medical facility.

3. Upon arrival at the medical facility, complete and sign the "Worker's and Physician's Report of Injury" form. Your healthcare provider will have this form, or it is available from the Industrial Commission of Arizona.

4. Ask the physician to immediately send the reports to the Industrial Commission of Arizona and to SCF Arizona so your claim may be filed.

5. Complete and immediately return all requests for information about your claim.
