Q. How do I pay for my insurance?

You will receive a mailed statement that itemizes your account. Send your payment via U.S. mail. Soon we will be adding other ways to pay.

Q. What’s an audit?
A.
With the implementation of our new computer system, the method for billing deposits and reporting payrolls changed. The policy audit will be required to reconcile and finalize each policy period of each account. If you have more questions, call 602.631.2600.

Q. I received a policy audit in the mail. What do I do next?
A.
The policy audit form must be returned so we can reconcile the account and apply the deposit to finalize each policy period. The reportable payrolls for the entire report period displayed at the top of the form must be entered under each job classification code shown. Policyholders may receive more than one policy audit form for each report period; all forms must be returned and completed to finalize the account. If you have more questions, call 602.631.2600.

Q. How do I file a First Report of Injury form?
A. Call 602.631.2300 or click here for a Form 101. The Form 101, once completed can be e-mailed as an attachment or faxed to SCF Arizona to initiate the claim setup. Soon, policyholders will be able to file this report through our website for faster service.

Q. Who can I call if I need claim information?
A. If you have a question about an injured worker’s claim, Claims Specialists in our Customer Contact Center can answer your questions. Just dial 602.631.2300 or 800.231.1363 between 7:30 a.m. and 5 p.m., Monday through Friday. Spanish-speaking agents are available at 602.631.2302.
